What these products actually do
Blood-flow creams and gels
The largest group. The active ingredients are usually vasodilators — L-arginine, menthol, niacin, sometimes horny goat weed extract — rubbed into the skin to draw blood into the surface tissue. The result is real but temporary: warmth, fullness, heightened sensitivity, lasting somewhere between twenty minutes and a couple of hours. That is a genuine effect and for many people it is the whole point. It is not structural change, and it does not accumulate over months.
Jelqing and conditioning serums
These are sold alongside manual exercise routines and pumping equipment. The serum itself is a lubricant with conditioning ingredients — its job is to let the skin move without chafing during a repetitive exercise, which matters, because friction injury is the most common thing that goes wrong. Whatever you make of the exercise, using it dry is the fastest route to a problem.
Oral supplements
Sold as food supplements, not medicines. In the United States that means the FDA does not assess them for efficacy before sale, and manufacturers cannot legally claim to treat a condition. Typical ingredients are amino acids, zinc, maca, tribulus and herbal extracts, and the defensible claim is general circulatory or energy support. If you have an erectile problem with a physical cause, a doctor has treatments that are tested and that work. Nothing on this shelf competes with that, and we will not pretend it does.
Using them without trouble
Patch test on the inside of the forearm first — menthol and niacin cause a stinging reaction in a minority of people, and finding that out on the genitals is unpleasant. Use a small amount and let it absorb before dressing. Keep every one of these away from the eyes and away from a partner’s mucous membranes. Most are not condom-safe if oil-based, so check before you rely on one. Do not apply anything warming or tingling internally unless the label explicitly says it is for internal use.
Drug interactions worth taking seriously
Several ingredients common in oral enhancement supplements affect blood pressure. If you take nitrates, alpha-blockers, or medication for a heart condition, speak to a pharmacist before starting one. This is the single genuine risk in the category and it is easily avoided.
Common mistakes
Expecting permanence from a temporary effect and then using far more than the label says. Skipping the patch test. Using an oil-based cream with a latex condom. Combining a numbing delay product with a stimulating cream, which produces a confusing result and blunts the feedback you need to avoid injury.
